The 32-year-old, who inspired Northampton to the 2000 European Cup when playing with a shoulder injury, was a cult hero at Newcastle helping them to gain promotion to the Premiership and win the title a year later.
However, he left three years ago after contract talks broke down and had a similarly influential impact on Northampton playing a leading role in their finishing second in the Premiership and culminating in the 9-8 win over Munster in the European Cup.
The New Zealand-born back rower, who came to prominence in the 1991 World Cup when the Samoans stunned joint-hosts Wales and played in the 1995 and 1999 editions as well, is now set for a key role when Newcastle start their European Cup adventure next season.
"Pat is a world-class player and a real favourite with the fans," said Falcons rugby director Rob Andrew.
"His return will add valuable experience and presence to our pack - he is still very hungry for success," added the former England fly-half.
